The application must be filled and information about your training facility, including name, date started and completed, course syllabus, and achievement record should be provided.



Exemption allows sitting for CNA test without going for CNA training again. Correct application should be filled and submit it to Bureau of Health Professionals or Nurse Aide Registry in your state.
